I'd argue it's more of an RPG than some mainstream games that are called RPGs nowadays.
You can go on lots of adventures with your family members. Just designate your sons/brothers/uncles as commanders and go to war. Or go on Viking raids. Or go Crusading. Or go looking for the Fountain of Youth.

Thea 1&2 are weird sandbox resource gathering RPG things, but one of the primary ways to get new bodies in them is for two characters to find a baby in a cabbage patch. The game doesn't actually track these relationships, but it's indeed possible to go adventuring with relatives if you remember they're such. In fact, in Thea 2 they start as children and it's beneficial to level them in the brief window before they grow up, so it's not uncommon to have parents aggressively striking out alongside their young charges.
Rogue Trader involves meeting very distant relatives briefly, and then dealing with dynastic squabbles within the house of your pet fish, and your butler's family rising above their station.

If bannerlord counts as rpg I guess with some mods to age up kids, marry anyone, enable kid combat.

I didn't say enjoyment in a single player RPG is dependent on the developer's story. To use the wizardry example above, the gameworlds of the latter installments Guardia and Dominos, did not have traditional tolkienesque races of dwarves, elves and hobbits native to the respective planets. Yet the game still allowed the player to make such characters. Why? The game never acknowledges those races, and they will never be encountered. But it is part of a free character customization system. As I said above, a player could create two characters, give them the same surname (thus represented in the game) and appropriate voice sets, and call them bickering brother and sister. It does not matter if the narrative reflects this or not. What you are dismissing as "imagining" is a customized backstory, and that is absolutely part of the role-playing game experience.
