I never played Morrowind and thought "Wow I need to mod in a way for NPCs to take a **** in outhouses."

Incidentally, we can use this NPC behavior to judge at a glance, right at the start of the game, how honest of a remake the remake is.

Plenty of games have reasons for it. I don't think the OP meant "do you need to be able to watch the **** come out of their anus"?

Characters going and using the restroom is part of a believable NPC simulation. Just lock the door when they go in, have the NPC stand there for a bit, then come back out. Done.

It all depends on context, I don't really get the point of the question tbh. I don't think it should be censored.
In a game like Hitman, NPCs going to the toilet provides an extra gameplay element.
In a lot of other games it would just be a kind of unnecessarily detailed immersion thing.
