The Firefly woman Joel kills says "You know she would choose this" (even though they weren't giving her a choice) and Joel also believes this which is why he lies to her instead of outright explaining that these people were crooked and clueless. In the first game it's outright spelled that the Firelies have no clue what they're doing, and the procedure will kill Ellie for nothing. In the end she sees through Joel's lie, but still decides to stay with him because she trusts him. The Firelies are the villains of the last arc, no better than the military or the various gangs that roam the wasteland.
You think they'd kill her for nothing based on what you're seeing, but this game was written by a guy who didn't know that there aren't any vaccines for fungal infections. He sure as hell wasn't thinking about the logistics of distribution. He writes for feelings.
