Main quest difficulty mod that the modtranny who made it says is compatible with MDMD

Its also completely pointless, if you're doing vanilla quests and dungeons, because everyone knows the locations of the best gear, the layout of most dungeons, the creature merchants, and how to cheese. HP would need to be tailored for all mobs to fit a groups damage range. well, the over all point of playing it multiplayer would be the fights themselves. I guess you could really get into it and start tailoring the quests, item locations, etc... to different things, changing the whole game up. This mod allows you to be "downed" and revived before bleeding out like other multiplayer games. Looks a little weird to install though.
