From what I've seen of it, it just looks like a multiplayer dungeon crawler rather than a MMO with a persistent world

It's pretty good. Each "quest" is its own dungeon, most of them are fully narrated by a DM. There's even a few narrated by Gary Gygax & Dave Arneson.

I thought the DM narration was neat and added a lot to the ambiance.
If I rated each quest on a 1-5 scale, I wouldn't place any of them below 3/5. And there's well over a hundred of these quests.

DDO is having a Hardcore event for the next 3 weeks, could be fun to see how far people can get. I would play on any regular server as well, re-downloaded and started looking at my old characters today.

Here is a graph of game activity. Hardcore (server) currently has the most players but it has free transfers even for characters that die.

hardcore is great if you like losing your character to lag
no, you will not avoid the lag, it's on their side. It's not frequent, but it doesn't need to be frequent to get you killed once.

DDO monetization is pretty despicable from what I've seen: they sell expansions, VIP, and has plenty microtransactions on top of it all.
