Yes. You should not be able to be a fatass and still have a small and nimble hitbox. I would go further and make sure that any fat characters run out of stamina quickly and have to catch breath regularly.
That does raise the concern (if we still had normal people making games) that for comp edge you just roll a female character (if hotbox sizes matter that is), but that's a topic for another time. Conversely, if what you need is a massive hitbox to get in the way of everything and everyone - well, maybe you do want to make a fatass then.

In everquest your hitbox is your model (or a close approximation) which means bigger characters have to crouch to enter houses in the hobbit area. Also, you can use shrink to get through some gated areas.

It really depends on the type of game. In Overwatch you have people have all different shapes and sizes but also attacks of all different shapes and sizes. You have conventional small bullets that Soldier 76 shoots which would have a decent chance to miss (his ultimate also gave him autolock on to adjust), but then you have Symmetra shooting big orbs or Reinhard swinging a humongous hammer in a wide arc or Mei with her fire extinguisher, etc. So hitbox size doesn't matter that much.
I think it could also be cool if only certain sized players can access parts of a map like a small hole or a vent. could make it important to find smaller players for your party for multiplayer quests or dungeons. Small characters should have huge hit boxes and fat characters should have tiny hitboxes. That's called fun.
That reminds of Dragons Dogma. If you made a very short character you could enter goblin holes in a certain area. I also thought it was cool how they incorporated body weight. Heavier characters could carry more items and withstand strong winds in one area of the map. The downside though was slower stamina Regen and slower movement.

A troll (literally) tried to charge an entry fee by blocking the way into sol b. He got in trouble with CS for doing that, but I couldn't help but admire his attempt.
@DecadeRiptide you play VT2 so you know the characters (obviously) have different hitboxes. The same is true in Darktide but besides Ogryns being fat and having stupid huge hitboxes, the selected height has an impact on both hitbox and camera height. There's a sweet spot for getting the maximum value out of most cover objects but it doesn't matter that much considering how mobile the game forces you to be.

I loved playing that game, but for some reason it got stuck on that Assassin mission where you had to track down 3 or 4 ( ? ) assassins at certain locations at certain times of day when they triggered but 1 or 2 would never trigger for me so I could never finish that mission, not sure if it was MQ critical or not but it always ****** me off, I couldn't get it done on 3 different characters. Maybe you know what quest I'm talking about. Anyone remember when they added the nali warcow to Unreal Tournament? People were caught off guard at first because they had no idea how to score headshots on the model . Then they figured out it was the turret on top which stood out like a sore thumb.

Well in game design you might make the men be stronger and tankier, and the women faster with more mobility. I also thought in real life, women generally have more stanima and was thought to be a survival thing. But at the same time I'm not so sure even a trained female would run 2km as easy as a trained male soldier could. I think you get too caught up on the "but balance" argument. Sure, competitive games probably do need some semblance of balance to be competitive, in which case just have only men, or only women compete and have your balance in something else, like kits.
If it's not a competitive game, there are arguments to be made that balance is not really as important as fun is, and for some people looks can be a part of fun. I ******* hate the modern gamer trend to ***** about balance in every game including ones where it doesn't belong, sometimes resulting in devs making arbitrary and lore breaking/nonsensical in the game world changes to accommodate the whiners. But really, it's ok, you can play a woman and have inferior stats across all physical categories, but you get to roleplay as what you picked (and stare, we know why you picked her). It's not like stats matter at the end of the day to such degree that a disadvantage will make things impossible for you. Even in MMOs with hardcore PvE content you can easily get away using non-optimized gear you found on the floor somewhere (as long as the ilvl is high) just fine, you don't need the best possible stats. It sure makes you gib harder, but you can live.
