Here are a few discrepancies that I've noticed related to Act 1 race swaps, along with a few other mismatches:
Baelen and Derryth Bonecloak in the Underdark are tagged as being humans, but are both still gold dwarves. The pair aren't listed as having been swapped in either table of name changes and Baelen was changed to have a proper dwarven beard, so I assume that this is simply an oversight.
Arron in the Druid's Grove was changed to be human, but his tag still says Lightfoot Halfling and he still has his original halfling racial traits.
Auntie Ethel in her initial appearance at the Druid's Grove is tagged as being a Fey rather than human as her disguise normally would be (she is flagged as human in her disguise form at her hut).
Olodan the Shadow Druid in the Druid's Grove seems to have an issue with floating earrings that don't properly connect to her head during the cutscene where she switches out of her rat form. While properly tagged, she and her fellow Shadow Druids, Pintaia and Wren, still have their original halfling racial traits rather than those of their new races (Olodan and Pintaia: human; Wren: Wood Half-Elf).
Lady Esther on the Monastery Trail was changed to be a half-drow, but her tag still lists her as human.
