Magic sex changes were canon from the 70s.
There are a bunch of ways for it to happen, all from curse magic.

This is OK for balance if you have like a 5-people party. 6 people is still lol, and no level cap in particular is lollercoaster. You will steamroll everything anyway.

There are overhauls designed around higher level caps and party size, but you can't install them midsave.

Consider instead https://www.nexusmods.com/baldursgate3/mods/6183 so you can walk around with a big party but only 4 of them fight.

To change biological sex you'd probably have to use time magic to retroactively replace someone completely with a different person. Your sex is emblazoned on every cell in your body and cannot be changed.

In a world where there weren't already too many ******** everywhere, it could be funny to do a comedy bit where some women is harassing a wizard begging him to change her sex so she can live as a man, while he tries to explain that he already did when the man who existed before asked for the same thing, which the woman version cannot comprehend because it was all retconned out of existence by the spell.

Well, in the Forgotten Realms, I'm sure one or more of the degenerate religions of some qweirdo deity offers spell transformation. Especially in Greenwood's base campaign.
Certainly any mid-level mage could also perform the "polymorph" spell (4th level, requires 7th level mage), although in some versions it has a chance to kill the target due to "system shock" (base 70% success at CON 10, adjusted by 5% each step, 97% at 17 and 99% at 18. Risky, but less so than trooning out).
But, as you all recognize, a significant subset of the ****** don't ACTUALLY want to be women. The AGPhreaks want to be a man in a dress offending people, so they would not use shapechange magic to transform completely.

Been playing this since I'm out of stuffs to play now, haven't really seen any woke trash except how gay Gayle is as he keeps flirting with my Astarion main, you'd need to be explicit just NOT to get anal'd. It's probably OOC tho since Astarion must be a *** considering how gay Vergil is but still.
Edit: Have they rebalanced the boss fights during the patch works? For example, in contrast to what people said, Otto's irresistible dance doesn't really work on Raphael since he now has a passive that cancels the spell when his turn ends. I still brute-forced him anyway, but it's quite a bummer.
Edit 2 : Didn't notice the rapefugee thing at Act 1 neither since those rapefugees just left after I committed Total Goblin Death.
