Morrowind was terrible about visual congruence in this regard, where you would visibly hit your target and be rewarded with a "miss" effect.

75% actually. It's only after that where you start taking stat penalties.
(Am I the only one annoyed that it took until Skyrim to start calling it "Stamina"? Fatigue is the absence of stamina. Having more fatigue should be bad.)
