| Solution: Patch made to give them red eyes |
So I just got to Act 3 to Fraygo's Flophouse where we meet Dalyria and Pale Petras, two of Astarion's spawn siblings. And the defining trait of someone who becomes a vampire spawn is that their eyes turn red. It wouldn't bother me if Astarion didn't have a scene where he explicitly states that. So while Dalyria is really pretty with those icy-blue-grey eyes of hers, they should be red. Not car-light glowy red, just plain red. All the spawn eyes should be, so that we can distinguish them from regular people.

I'm a little bit further into Act 3 now and just had the spawn visit camp. These are Leon and Aurelia. Leon also has non-red eyes but should. Aurelia was turned into a drow, and she does have red glowing eyes, just fyi.

Does Astarion really have dialogue confirming vampire spawn = red eyes? I think the philosophy was they’d have red eyes during the ritual and normal eye colors throughout the rest of the game

Darling, you doubt me?
Don't worry, I've got you on all things Astarion.

Consulted with
@Silver about how the characters were edited.
TLDR: The eye colors of the vampire spawn is intentional.
Non TLDR:
- Forgotten Realms lore says that "Vampire spawn appear just as they did in life, although their features are often hardened and feral, with a predatory look." [1]
- Since we can't make them have "hardened and feral" features, we restored their eyes to the color that would match their race.
ㅤ
- Dalyria who is a high elf has high elf purple eyes. The rest who are human have human shades of eyes
ㅤ
- Larian already broke FR lore in their depictions of vampire spawn, so we are working around it to fit the story and role they have. Logic, I think, is RR has spawn have normal eyes when they're well fed, whereas spawn in the cells have bloodshot deep red eyes because they're malnourished.
- The reason they don't all have red eyes is because they are supposed to be predators seducing prey. Red eyes would be off putting and strange to regular BG citizens and be a dead giveaway that they're vampiric. RR makes all the well-fed and favored vampire spawn have regular eye colors so they can better fit that predator role
ㅤ
- Astarion doesn't necessarily confirm all spawn have red eyes. He says he doesn't remember what shade his eyes used to be, meaning some flexibility I guess? I suppose you could also argue he isn't "well fed" by time we meet him, or his eyes are actually naturally red and he doesn't realize. Something like that. In a perfect world, he wouldn't have said that line and have different eye color to better match his backstory
I understand this is something subjective so I'd be willing to make a patch to give them red eyes, but that's why they appear the way they do currently

I would argue that the other spawn aren't well-fed either.
Eyes turn red when you are turned into a spawn.
Can I have a patch, please? *puppy eyes* Maybe I should figure out how to make one...

Since you said that, I will let you make a patch!

Become a modtroon!
Let me know if you need help on how to do it. Start by installing BG3 Modding Toolkit and I'll set up a project

Eeep! Me and my big mouth... Leaving aside that I have a full-time job, I really don't have time to mess with this right now (I have two pseudo-parallel playthroughs to finish - it's complicated and silly but I'm enjoying it a lot.. maybe you'll end up watching both of these game-movies and notice what I was doing there), and I'm almost done with the first of them - and then I'll have to edit it and upload it... and that one will be hours long. I'm dreading it already...
Maybe I'll spontaneously get back to you though. You never know what my brain will decide to start obsessing about.

After the ritual, Dalyria's eyes got glowy-red. I'll assume that the tacky glowiness wasn't intended here, especially since she's the only one of them who has them in that scene. From the six spawn, the only one who isn't captured by the camera is Aurelia - the drow that already had red glowy eyes when she popped up at camp with Leon in order to abduct my vampire boyfriend.

And here's a stabbed and dying Cazador who gets purple-faced as soon as Astarion prepares to stab him ("But I'm not above enjoying this.") The spots appear on non-RR Cazador too, but the colouration is a little bit less glaring:
With RR:
No RR:
Cazador has glowing red eyes through all of this, but that's fine for the big bad boss dude, I'd think.
